The Motor Salvage Operators Regulations
set out what
information must be gathered from each applicant when they apply for
registration and what information must be placed on the register, but there is
not a standard application form.
A number of local authorities have given permission for their forms to be
viewed via this toolkit to help anyone who wishes to revise or further
develop their own forms.

There are many other examples, but these organisations have used these forms successfully in their registration/vetting process and have gone on to take enforcement action.
